Frequently Asked Questions

If you are insured by an HMO or in some cases a PPO, your health plan will likely require a doctor’s referral for specialty medical care. This ensures you have a primary physician that is your main point of contact for all of your health needs, which helps maintain continuity of care and a secure location for your confidential medical files.

To ensure you are seen quickly once you arrive and that your time with the physician is used effectively, it will be helpful if you download, print and complete our new patient medical forms. Also, please remember to bring your photo ID, insurance card and a list of any medications you’re taking.
